The most convenient time to contact
the doctor for advice is during morning surgeries between 9.00 and 11.30am, where
you can speak to a GP on call for that day.

MAKE
AN APPOINTMENT?
Consultation with the doctors
and the practice nurses are by appointment. Appointments can be made by telephone
or by calling at the surgery. Urgent cases will always be seen the same day. You
can make an appointment with any doctor you choose. However, it is best to see
the same doctor through the course of each illness.
If you find that you cannot attend
for an appointment, please telephone and cancel, even at short notice, as we can
often offer your cancelled appointment to somebody else. Repeat prescriptions,
medical forms and certificates can only be dealt with through a booked appointment.
You will not be seen as an emergency appointment.

TEST RESULTS?
The results of tests take about one
week. Please wait this period before coming to the surgery and arranging to see
a doctor. Some results can, however, be passed over the phone - ring the surgery between
2.00 and 3.00pm, and ask to speak to a nurse.

Please notify us of any change of
address or telephone numbers so that we can bring your records up to date. You
will need proof of your new address, so please bring a copy of a utility bill in
your name.

OBTAIN
A REPEAT PRESCRIPTION?
We offer a repeat prescription
facility at the discretion of the doctors. Allow 48 hours from date of request
using the official repeat prescription form. Patients requiring a review of their
medication will not be seen as an emergency appointment.
Please note:
if you see a doctor as an emergency, only two weeks' supply will be given.
HOW DO I...
REGISTER WITH THE PRACTICE?
The registration
desk is open Monday to Friday from 9.30 to 10.45am. Patients who wish to register
with our practice need to ring from 8.30am on the day they wish to come to register,
to confirm whether the registration desk is open, as sometimes, due to staff shortages,
this cannot be guaranteed.
Documents required when registering
with a GP
If it is your first time of registering with a GP in
this country, you will need:-
- Your passport - with a visa valid up to six months.
- Proof of your current address (eg tenancy agreement or recent utility bill).
- Your birth certificate.
- Your date of arrival in the UK.
- Telephone or contact number.
If you are already registered with the NHS
but are coming from another practice, you will need to bring:-
- Proof of your current address (eg tenancy agreement or recent utility bill).
- Your birth certificate.
- Your medical card or, alternatively, the name and address of your previous GP.
- Details of previous address.
- Telephone or contact number.
